Health and Wellness

Holy Family Parish is proud to partner with OSF in the Faith Community Nursing Program.  The Faith Community Nursing program offers a unique partnership between two healing entities; our hospitals and the faith community. Faith Community Nurses partner with clergy and healthcare organizations to advocate for the health and well-being of the congregation. While the focus of the program is on preventative healthcare, some of the ways members are helped to lead healthier lives is through education, screenings and referrals to community resources. Faith and Health

  • Promotes understanding of how faith and health are related
  • Assists others to improve their spiritual and physical health through prayer and support

 

Health Education

  • Monthly educational materials to be used for bulletins, educational boards, and/or programs
  • Health fairs
  • Coordination of health educational events on site on a wide variety of wellness and disease specific topics
  • Free education for Advanced Care Planning/Power of Attorney for Health Care offered to your congregation

 

Health Advocate

  • Assistance in navigation of the health care system for specific individuals in need
  • Wellness screenings
    • Blood pressure
    • Diabetes
    • Biometric testing
    • Health risk assessment
    • Depression
    • Sleep apnea
  • Hospital, nursing home, and home visits as needed
  • Coach about medical appointments or attend appointments as requested
  • Referrals accepted for health advocacy
  • Assistance in creating a volunteer network within your church community

 

Health Counselor

  • Assists with the development of a wellness council
  • Assesses church community needs
  • Liaison to community resources
  • Support from OSF Saint Francis Medical Center Faith Community Nurse Steering Committee with opportunity to participate
